NANCA (antineutrophil cytoplasmic antibodies) - autoantibodies directed against antigens present in the cytoplasm of granulocytes Neutrophils. ANCAs are present in many diseases immunological diseases, and can also occur in healthy individuals. Antigens for ANCAs include proteinase 3 (PR3), myeloperoxidase (MPO), BPI (bactericidal permeability increasing protein), cathepsin G (KT), elastase (EL), lactoferrin (LF), lysozyme (LIZ), azurocidin (AZ), enolase (EN), actin (AK) and others. Antibodies against proteinase 3 (PR3 - ANCA) are helpful in the diagnosis of granulomatosis with vasculitis. They are present in 80 - 90% of of patients and their titers correlate with clinical status. PR3 - ANCA are also found in microscopic vasculitis (in 15 - 45 % of patients) and in vasculitis 45% of patients) and in vasculitis limited to the kidneys (25 - 36%). Antibodies to myeloperoxidase (MPO - ANCA) are detected in microscopic vasculitis (in 45 - 80% of patients), vasculitis confined to the kidneys (40 - 65%), and in a small percentage of percentage in Wegener's Granulomatosis (5 - 15%), as well as in drug-induced immune reactions drug-induced immune reactions and in Goodpasture's syndrome. W chronic immune-mediated liver diseases ANCAs have also been found, mainly antibodies directed against BPI and cathepsin G.
