PTH intact

PTH parathormone - a polypeptide composed of 84 amino acidss, secreted by the parathyroid glands. It has a variety of actions, the most important being against hypocalcemia. Parathyroid cellsregistering plasma calcium concentrations via calcium receptorsincrease the secretion and release of PTH into the circulation.

PTH increases the absorption of calcium in the plasma

PTH increases the reabsorption of calcium in the renal tubules and intestine, and causes the mobilization of calcium and phosphates from bone via bone resorption. PTH also inhibits the reabsorption of phosphatein the renal tubules, resulting in increased loss of phosphateby the kidneys .

PTH activates vitamin D 1α-hydroxylase in the renal proximal tubule, whichra responsible for the synthesis of 1,25 (OH)2D, resulting in increased intestinal absorption of calcium and phosphatew. PTH is a perishable molecule, 25% of circulating PTH is the unchanged form of iPTH, 70% is C-terminal fragments, 5% N-terminal fragments.

Recommendations for PTH determination include osteopathies, diagnosis of rs hyper and hypocalcemia, renal failure, nephrolithiasis, suspected hyperparathyroidism, malnutrition syndrome or parathyroid adenoma.

It is advisable to take in the morning, fasting, with a 12-14 hour fast.
